In my opinion, the best way to care for a Hydrangea is to just leave it alone. It will survive winter just fine on its own because it is quite hardy. In fact, all of the foliage will die back to the ground this winter and grow back up from the ground next spring. And yes, it flowers on old stems. In fact, if you trim your Hydrangea in spring, you will have cut all of the flower buds off. So, if you can, I suggest to not trim the Hydrangea.
